• Form of payment must be Check or Money Order

Requirements:

– 1 Passport quality photo is required (unavailable at our location)

– 2 Certified copy of Birth Certificate or Naturalization Certificate

– 3 Drivers License or Photo ID

– 4 If under 16 yrs of age, both parents or legal guardians must accompany the child when applying. Both parents or legal guardians must sign the application for the child. If this is not possible, the single applying parent or legal guardian must show proof of sole custody or the consent of the second parent.
